Waiver of Liability, Assumption risk, Permission to Treat

 

1. The risk of injury involved in  participation of the listed sport & coaching services, significant, with the potential of permanent paralysis or death, and although established rules, equipment, and personal discipline may reduce the risks, the risk of serious injury still exists.

2. I knowingly and freely assume all risks listed above, both known and unknown, and assume full responsibility for my participation.

3. I acknowledge that this is my responsibility to understand and obey instructions and rules set by the medical staff for participation and use of equipment. If an injury is sustained during participation, I understand that I am in the care of Brown Bambi LLC coaching staff, and willingly give permission to receive treatment and care by those hired, and will follow designated protocols in place for return to play. If I have any questions, I will seek help from my personal medical physician.

4. I willingly agree to comply with the instructions, policies, and terms and conditions by all aspects, including the coaching staff and my personal medical staff. If I observe any unusual hazard during participation, I will remove myself from the situation and bring it to the attention of the nearest coach immediately.

5. I understand that if I sustain any injury that requires referral to a physician or the emergency room. My primary insurance will be utilized to its fullest. Return to physical activity clearance must be communicated via physician.

6. I understand that if I choose to drive my own vehicle or am a passenger of a Brown Bambi LLC vehicle while travelling to and/or from any sporting event I, for myself, assume full responsibility for any risk of loss, property damage or personal injury, including death, and hereby indemnify, release, and hold Brown Bambi LLC, and their, officials, coaches, medical staff; other facility employees including maintenance and custodial; and other sport participants harmless with respect to any and all injury, disability, death, or loss or damage to person or property, to the fullest extent permitted by law.
    

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
